GLP-1 Long-Term Side Effects: What Studies Show After 2+ Years

Most side effect discussions focus on the first few weeks of GLP-1 use. But what happens after one, two, or five years? Research from large clinical trials and long-term follow-up studies is beginning to answer that question — and the picture is more nuanced than either supporters or critics suggest.
Short-term side effects of GLP-1 medications — nausea, constipation, vomiting — are well documented and usually fade within the first few months. What researchers and patients are increasingly asking about is what happens over the long haul. As GLP-1 drugs move from short-term prescriptions to decade-long therapies for chronic weight management, understanding the multi-year safety profile has become essential.
The good news is that we have more data than many people realize. Semaglutide (Ozempic/Wegovy) has been studied for up to five years in the SUSTAIN and STEP trials. Liraglutide (Saxenda/Victoza) has real-world safety data going back to 2010. Tirzepatide (Mounjaro/Zepbound) is newer but has completed multiple 72-week and 88-week trials. Here is what the evidence says.
Gastrointestinal Side Effects Over Time
The most common side effects of GLP-1 medications are gastrointestinal: nausea, constipation, diarrhea, vomiting, and abdominal discomfort. The reassuring finding from long-term data is that these almost universally improve with time. In the STEP 5 trial — a two-year semaglutide study — gastrointestinal adverse events were most frequent during the dose-escalation period in the first 20 weeks, then declined significantly and remained low through week 104.
However, a subset of patients experiences persistent constipation throughout their time on these medications. Chronic constipation on GLP-1 therapy can contribute to hemorrhoids, fissures, and — in rare cases — fecal impaction. Long-term users should ensure adequate fiber intake (25–35 g/day), hydration, and physical activity. If constipation does not resolve, discuss stool softeners or fiber supplementation with your doctor.
Thyroid Concerns: C-Cell Tumors
One of the most scrutinized long-term safety signals for GLP-1 drugs is thyroid C-cell hyperplasia and the theoretical risk of medullary thyroid carcinoma (MTC). This concern originated from rodent studies showing that high doses of semaglutide and liraglutide caused dose-dependent C-cell tumors in mice and rats.
Critically, rodents have far more GLP-1 receptors on thyroid C-cells than humans. Multiple human pharmacology studies and post-market surveillance reviews have not found a statistically significant increase in MTC in humans taking these medications. The FDA requires a black box warning about MTC on GLP-1 drugs and contraindication in patients with a personal or family history of MTC or Multiple Endocrine Neoplasia syndrome type 2 (MEN2). Outside of these high-risk groups, the current evidence does not suggest GLP-1 medications cause thyroid cancer in humans — but monitoring continues.
Pancreatitis Risk
Early post-marketing reports and some smaller studies raised concern about a possible link between GLP-1 receptor agonists and acute pancreatitis. Large outcomes trials — including LEADER (liraglutide), SUSTAIN-6 (semaglutide), and SURPASS-CVOT (tirzepatide) — with combined follow-up of hundreds of thousands of patient-years have not confirmed a statistically significant increased risk of pancreatitis compared to placebo.
A 2024 meta-analysis in JAMA Internal Medicine pooled data from 22 randomized trials and found no significant association between GLP-1 use and pancreatitis. However, because obesity itself is a risk factor for pancreatitis, and because sporadic cases do occur in GLP-1 users, clinicians still advise patients to report persistent, severe abdominal pain radiating to the back, as this could signal pancreatitis requiring immediate evaluation.
Muscle Loss: A Growing Concern
Perhaps the most actively discussed long-term concern among GLP-1 users is lean muscle mass loss. GLP-1 medications produce rapid weight loss, and a substantial portion of that weight comes from lean tissue rather than pure fat. In the STEP 1 trial, approximately 39% of the total weight lost by semaglutide users came from lean mass. At a 15% body weight reduction, that can represent several kilograms of muscle.
Over the long term, muscle loss can reduce basal metabolic rate, increase the risk of sarcopenia (age-related muscle decline), and impair physical function — particularly in older adults. The evidence-based countermeasure is resistance training combined with adequate protein intake (at least 1.2–1.6 g of protein per kilogram of body weight daily). Some researchers are also investigating whether co-prescribing creatine or optimizing protein timing can better preserve lean mass on GLP-1 therapy.
Bone Density and Fracture Risk
Rapid weight loss of any cause is associated with reduced bone mineral density, and GLP-1-driven weight loss is no exception. Data from the STEP 1 extension and from bariatric surgery literature (which produces similarly rapid weight loss) suggest that total hip and lumbar spine bone density can decline 1–3% over two years of major weight reduction.
The fracture risk from this degree of bone loss is modest for most patients but may be clinically meaningful for postmenopausal women, older men, and those with pre-existing low bone density. Weight-bearing exercise, calcium and vitamin D supplementation, and DEXA scans (bone density tests) at baseline and after 1–2 years of therapy are reasonable precautions for at-risk individuals.
Gallbladder Disease
GLP-1 medications are associated with a meaningfully increased risk of gallbladder-related complications, particularly gallstones (cholelithiasis) and gallbladder inflammation (cholecystitis). Pooled analyses of semaglutide trials show approximately 1.5–2x higher rates of gallbladder disease compared to placebo, a pattern also seen with rapid weight loss from other causes.
The mechanism appears to be twofold: rapid mobilization of cholesterol into bile during weight loss, and reduced gallbladder motility due to GLP-1 receptor activity. Patients who develop right-upper-quadrant pain, especially after fatty meals, should be evaluated for gallstones. For most, this is a manageable complication — but it is the most consistently demonstrated long-term adverse event unique to GLP-1 pharmacology.
Kidney Function
The long-term data on kidney effects of GLP-1 medications is largely positive. The FLOW trial — a dedicated kidney outcomes trial of semaglutide in patients with type 2 diabetes and chronic kidney disease — was stopped early in 2024 because semaglutide showed a highly significant reduction in the composite endpoint of kidney function decline, kidney failure, and cardiovascular death. This represents one of the most important long-term benefit signals for this drug class.
Despite the overall beneficial renal picture, some patients on GLP-1 therapy develop acute kidney injury (AKI), typically related to dehydration from vomiting and diarrhea. This is most likely during the dose-escalation period. Staying well-hydrated and knowing when to seek care for persistent vomiting are practical precautions.
Cardiovascular Benefits as Long-Term Safety Context
It would be incomplete to discuss long-term side effects without acknowledging that GLP-1 medications have demonstrated substantial long-term cardiovascular benefits. The SELECT trial showed a 20% reduction in major adverse cardiovascular events (MACE) with semaglutide in people with obesity and established cardiovascular disease. The LEADER trial showed similar benefits with liraglutide.
These benefits compound over time — the longer the follow-up in cardiovascular outcomes trials, the larger the benefit tends to be. For high-risk patients, the long-term cardiovascular benefit likely outweighs the side effect profile described above.
Weight Regain After Stopping
While not a traditional "side effect," the weight regain that occurs when GLP-1 medications are discontinued is a critical long-term consideration. The STEP 1 Extension study found that participants who stopped semaglutide after one year regained approximately two-thirds of their lost weight within the following year, and many metabolic improvements reversed as well.
This finding has led many clinicians to reframe GLP-1 therapy as a chronic condition treatment — similar to antihypertensives or statins — rather than a finite course. The implication is that the "long-term side effect profile" is increasingly relevant to nearly all GLP-1 users.
What to Monitor During Long-Term GLP-1 Use
- Body composition (muscle vs. fat mass) — DEXA or body composition scales every 6–12 months
- Bone density — baseline DEXA for at-risk patients; follow-up at 1–2 years
- Gallbladder symptoms — report any right-upper-quadrant pain to your doctor
- Thyroid — self-examine for neck swelling; report any hoarseness or dysphagia
- Kidney function — basic metabolic panel as directed by your provider
- Protein and micronutrient intake — GLP-1 users often eat significantly less food; nutrient deficiencies can develop over time
- Blood glucose — GLP-1 medications can cause hypoglycemia in diabetic patients, especially those also taking sulfonylureas or insulin

The long-term side effect profile of GLP-1 medications is broadly manageable and, for most patients, favorable compared to the risks of untreated obesity. The most clinically significant and consistently documented adverse events over time are gallbladder disease, lean muscle mass loss, and modest bone density reduction. Thyroid and pancreatitis risks have not been confirmed in human long-term data despite initial theoretical concerns.
The key to safe long-term GLP-1 use is not avoiding the medication but pairing it with appropriate monitoring, adequate protein and micronutrient intake, resistance exercise, and regular check-ins with your prescribing clinician. The research base is still growing — ongoing trials with 5- and 10-year follow-up will refine these conclusions further.
