Understanding the Different Ozempic Pens
Ozempic comes in three different pen presentations, each delivering a specific dose range. It is important to know which pen you have, as they are not interchangeable.
- 0.25 mg / 0.5 mg pen (red label): Used during the initial titration phase. Each pen contains 4 doses of either 0.25 mg or 0.5 mg. The dose is selected using the dose counter.
- 1 mg pen (blue label): For patients on the 1 mg maintenance dose. Each pen contains 4 doses of 1 mg.
- 2 mg pen (yellow label): For patients on the 2 mg dose. Each pen delivers the full 2 mg in a single injection.
Before You Begin: Preparation Steps
Proper preparation before injection reduces discomfort and ensures the medication is handled safely.
- Remove the pen from the refrigerator 30 minutes before your planned injection time to allow it to reach room temperature. Cold medication causes more stinging.
- Check the expiration date on the pen label. Do not use an expired pen.
- Inspect the medication through the dose window. It should be clear and colorless. Do not use the pen if the medication appears cloudy, discolored, or contains particles.
- Wash your hands thoroughly with soap and water.
- Gather your supplies: the Ozempic pen, a new pen needle (NovoFine or NovoTwist, 32G, 4 mm recommended), an alcohol swab, and a sharps disposal container.
Step 1: Attach the Needle
The Ozempic pen does not come with a needle attached. You attach a new, sterile pen needle before each injection.
- Remove the pen cap by pulling it straight off.
- Wipe the rubber stopper at the top of the pen with an alcohol swab and allow it to dry.
- Take a new pen needle and peel off the protective paper tab.
- Push the needle onto the pen tip and turn it clockwise until it is firmly attached (usually about 6 turns). Do not overtighten.
- Pull off the outer needle cap (save it for later needle removal) and then remove the inner needle cap. Discard the inner cap.
Step 2: Prime the Pen (New Pen Only)
Priming is required only when you are using a new pen for the first time. It removes air from the needle and cartridge system and ensures accurate dosing.
- Turn the dose selector to the "flow check" symbol (two lines on the dose counter).
- Hold the pen with the needle pointing upward.
- Press and hold the dose button until "0" appears in the dose window. A drop of semaglutide should appear at the needle tip.
- If no drop appears after the first attempt, repeat the priming step up to 6 times. If still no drop appears after 6 attempts, do not use that pen — contact your pharmacy.
- Priming is not required for subsequent injections from the same pen.
Step 3: Select Your Dose
Turn the dose selector to your prescribed dose. The dose counter will show your dose amount in the window (e.g., 0.25, 0.5, 1, or 2). You will hear and feel clicks as you turn the selector. Do not press the dose button while selecting your dose.
- If you turn past your dose, you can turn it back.
- Do not attempt to select a dose larger than the remaining amount in the pen — the selector will not move beyond what is available.
- Confirm your dose in the window before proceeding.
Step 4: Choose and Prepare the Injection Site
Select your injection site: abdomen (at least 2 inches from the navel), front of the thigh, or outer upper arm. Clean the site with an alcohol swab and allow it to dry completely — at least 10 seconds. Injecting before the alcohol dries causes unnecessary stinging.
Step 5: Inject the Medication
- Using your non-dominant hand, pinch a fold of skin and subcutaneous fat at your chosen site.
- Hold the Ozempic pen with your dominant hand, like a dart.
- Insert the needle into the pinched skin at a 90-degree angle (perpendicular to the skin surface) with a smooth, firm motion.
- Once the needle is fully inserted, press the dose button all the way down with your thumb.
- Keep the dose button pressed down and count slowly to 6 (six full seconds). This ensures the complete dose is delivered and prevents medication from leaking back out when the needle is withdrawn.
- After 6 seconds, release the dose button. The dose counter should show "0" to confirm the full dose was delivered.
- While still pressing the dose button (or just after releasing), withdraw the needle smoothly at the same angle it was inserted.
- Release the skin pinch after withdrawing the needle.
Step 6: After the Injection
After withdrawing the needle, do not rub the injection site. Light pressure with a cotton ball or gauze is fine if there is a small amount of bleeding, but rubbing can cause irritation and affect absorption.
Step 7: Dispose of the Needle Safely
Needle safety is important for your protection and for others who may handle your waste.
- Replace the outer needle cap over the used needle using a one-handed scooping technique — do not recap with two hands, as this risks needle stick.
- Unscrew the capped needle from the pen and drop it directly into your sharps container.
- Never throw loose needles in household trash. Use an FDA-cleared sharps disposal container.
- Replace the pen cap on the Ozempic pen.
Storing the Pen After First Use
After the first use, an Ozempic pen can be stored at room temperature (up to 77°F / 25°C) for up to 56 days, or returned to the refrigerator. Store the pen without a needle attached — always attach a new needle immediately before each injection. Never store the pen with a needle attached, as this can allow air into the cartridge and increase the risk of infection.
Checking the Dose Window and Troubleshooting
Dose Counter Did Not Return to Zero
If the dose counter did not return to zero after pressing the button for 6 seconds, you may not have received your full dose. Do not inject again. Contact your prescriber or pharmacist for guidance. Note how much dose remains in the counter so they can advise on whether you need a replacement dose.
Air Bubbles in the Cartridge
Small air bubbles in the cartridge are normal and will not cause harm if injected subcutaneously. They do not affect dose accuracy significantly. If you see large air bubbles, hold the pen with the needle pointing up and gently tap the pen to move bubbles upward before injecting.
Medication Leaking at the Injection Site
If you see a drop of liquid at the injection site after withdrawing the needle, you may not have held the button down for the full 6 seconds, or you may have withdrawn the needle too quickly. For the next injection, ensure the 6-second hold is complete before withdrawal.
